Done after multiple IUI failures, or directly if female has blocked tubes, moderate-severe endometriosis, ovulation disorders, ovarian failure, severe male factor.

In this, controlled ovarian hyperstimulation (COH) is done, where from Day2/Day3  the female is given daily injections to form multiple eggs. The growth of eggs is tracked by ultrasound. When the eggs reach around 18-20 mm size, last injection called trigger is given to mature them and then after 34-35 hours of trigger injection, the eggs are retrieved transvaginally under anaesthesia. It takes barely around 20 minutes for the procedure. The same day husband is asked to give his semen sample. The semen and eggs are then cultured in laboratory following which next day embryos are formed. The embryos are then cultured in lab for maximum up to 5 days (depending on quality). The embryo transfer is then done accordingly on Day2/day3/day5 of embryo growth. Day 5 embryo is called a blastocyst and pregnancy rates are improved further with a blastocyst transfer. After 14 days of embryo transfer, the couple is asked to get a pregnancy test done.
